Heads up, your headerinclude may be missing this (between the lang lines and var lines):

var templates = {
 modal: '{$jsTemplates['modal']}',
 modal_button: '{$jsTemplates['modal_button']}'
 };

Without this, any actions that will pop up a modal dialog will probably not work. I had a problem with removing attachments. Report and rating will pop up a modal, and they may need the above to work.
