Add a html element (placeholder) into PHP

I'm trying to change around some text box elements for my Admin CP to include a placeholder, but I'm not fully confident on which php file I need to change for it to work.

if you are talking about the admin login:

admin -> inc -> class_page.php

From line 386.

Remember to make a backup of the file before you change it.
