[Request] New Posting Template..

Hi there.
I need a new posting template for a specific forum.
What it looks like:

Username : Their username
Link To File : Links Goes Here
Extra Info : That goes in here
Images : Image Location In Here

And then when you press Submit, it gets posted by a username "Downloads Database", with the context like this:

Username:
File:
Extra Info:
Images:

And also, the post will go up into the moderator queue.

Is this possible?
I had a think about some of it, and you could pull the username variable, make a $file $info and $images variable.
But I'm new to PHP, so I couldn't code it.
