Is there anyway to make viewers click a box to view a thread, to state they are over 18 for instance.

You can always add it to the registration agreement when new users register.

You can edit the agreement by going to: Admin CP > Configuration > Languages > Edit Language Variables > member.lang.php

Look for agreement_1, 2, 3, 4 and 5 and you can include or alter those texts to suit your needs - i.e. must be 18+ or older to register, etc.

I already have the over 18 rule on sign up, but I was thinking more on threads and posts though. To make a thread available for sexual explicit content, (something i don't allow on normal threads)  but have to click a warning first to open the thread - kind of thing.

OK solved it. I just made a new group and an new forum where the new group can post too. Its moderated to join so some kind of protection i guess. No one outside of the group can see whats inside it so its all cool, and didn't need to plugin or mod.
