Team Live Chat Install Error

Team Live Chat will not install & activate.
I get the SQL error page that tells me to read the documentation and visit these forums for help.

I have included a screenshot of the page I get.

Any help or information on why this is happening would be greatly appreciated.

Turn on error handling in the ACP to find the detailed error.  That message is generic - and somewhat useless.  This post illustrates how to turn on logging errors, but there're many others posted.  Main thing, turn on error handling to display causal error:
