A couple of questions

thats what i had my cookies as but i cant logout properly
i have tried all these setting in all combinations

domain : .killercarrot.co.uk
killercarrot.co.uk

/forum
/forum/

try no forward slash at the start of forums

eg:
path: forum/
--or-- forum

And after you change it, clear your cookies.

Seems to work for me now, after setting it to:

.domainname.com

/forum/

and then deleting my cookies.

The reason why the cookies arent being unset is because it still sets the expire time to 1 year and not setting it to minus one year

$expires = time() + (60*60*24*365); // Make the cookie expire in a years time

where is should be for logging out

$expires = time() - (60*60*24*365); // Make the cookie expire in a years time
