3. If it only happens inside [code] tags, are [php] tags also affected?
hmm change your theme to defualt and check it.
sounds to be tags surely some code has affected , just bring out the recent modifiications or plugins installed.

Yes it happens in PHP tags as well.
I guess it is an error with the "Javascript" blocking with HTML enabled forums then...
This bug has been fixed in the latest code.
Please note the latest code is not live on the site or for download. An update will be released which contains this fix.
