Martec. I've tested both of your mods. The one without description fails to add the "s" button to the menu. I've tried making this mod twice, each time on fresh files. With all tests the image file and template mod were in place.
It looks to me like the only differences between the two versions occurs in bbcodes_sceditor.js. But I don't know jscript and so I cannot spot anything.
