In my original post I had a picture of a pm sent by system for new messages.  The MyBB engine by default.  So if the "system" doesn't have the custom profile fields included in the message the images are broken.

Oh rite. is system something that sends out PM's automatically like a bot?

It should keep the default structure still if it's a default feature. I didn't even know it existed so not really sure about it.
