I'm sorry but I don't know where is problem if user activation mail is working.
Have you changed any core files or templates recently? And when your problem firstly appeared at your site?

For registration? What version of MyBB you have, have you updated it or changed some of it's files recently?
Also, check in that Log when first bug appeared please. That could help.
My version is 1.8.14
i updated it 3 days ago.
Please try to disable all your plugins and than try requesting password reset or registration.
I did that, it did not work.
