Your theme doesn't seem to have the latest templates. You need to revert showthread_poll_option_multiple at least. You may also have other templates out of date.
showthread_poll_option_multiple as of 1.4:

Yours has: value="yes" which suggests 1.2.x

Whilst you're there, make sure all of your other template are default. Some may be edited because of plugins but you might want to deactivate them, revert all your templates and activate your plugins again. You might get more errors if your templates aren't as they should be.
Cheers! I suggest you revert any other outdated templates since old 1.2.x based templates may cause some conflict with 1.4 operation.
