Have you reset their password too?? Something must be changing here, things wouldn't randomly start not working on their own and then start working again on their own.

Sorry, I honestly have no idea. It's a very sporadic issue, works one minute then doesn't, I'd say it was the theme as it's only breaks for me on your custom theme, but I don't know what it'd be as the source of the form looks fine.
