I'm using a fixed width.

First make sure all the files have been properly uploaded to their respective locations, if needed re-upload them.
Next, deactivate and activate the plugin. If that doesn't work, I'll need a temp admin account to have a look at it, you can PM me that.

I've tried enabling this on my forum, but for some reason it often displays the yellow "click this to show full image" twice?

How can that be?
