There is the fix for your problem and it was writen many time ago.

As i said if you use a plugin to format names because mybb does not do this by default.

If you have maded via core files you have to know that i have seen some time ago a core bad function written for this purpose that consumes a lot of queries.

This plugin way only takes one and rewrite firmated names by cache instances.

Your problem can be as i said in my past message a conflictive plugin and you can be sure of that disabling that feature due it is not called anymore to do the job, i have written that option because of that posible issue. So, now you can see it that all was writen in order to fix posible issues.

Hi, I add an avatar to the last posts, but it breaks.

With an image is difficult to know about that issue i can make an image too with the fixed issue but it is due your skin, use default mybb skin and let me know.

Anywaus you can see it workin into my forums and that way is how to look like.

Is it possible to add Avatars on postbit ?

if user has not set any avatars then postbit will show default avatars. but i want to use  Lastposter avatar plugin show avatars on postbit if user has  not set  any  avatars.

^ IMO, that confuses your forum members !
perhaps it would be better to make a nice looking default avatar for your forum & use it ..
