infernoshout.php goes in your forum root.

The shoutbox just grabs the profile picture using the Mybb system, so the two simply aren't compatiable, at least I imagine not without a decent amount of work.

Please keep any plugin support inside of the actual plugin thread, there is no reason to go spamming the mybb forums when you can just post your question here.

That means the CSS file isn't being loaded. Assuming you are using my update for 1.8, check under your theme and make sure that alongside global.css and several others, there is a one called inferno.css. If that file doesn't exist, you can either create it and paste in the contents of /inc/plugins/inferno/inferno_style.css or uninstall it and reinstall it.

Also, I'm not sure what went wrong, since I've upgraded my board to 1.8.3 and it still works fine.
Gonna set up my new forum in a few weeks.
About when approximately could we expect the official 1.8 version?

I don't know. It's not exactly easy re-writing a plugin this large. It will be done when it's done.
